Vulnerability Remediation and Regression Testing: The Step Most Teams Skip

Vulnerability remediation regression testing is the practice of running a full or targeted regression test suite after every vulnerability fix, before promoting the change to production. It ensures that patching one security flaw has not broken existing functionality. Skipping this step is one of the most common causes of post-remediation incidents in enterprise applications.

Why Does Vulnerability Remediation Cause Unintended Regressions?

Every vulnerability fix is a code change - and every code change carries regression risk. The problem is that the urgency framing around security issues causes teams to bypass the test controls they apply to every other change.

In our experience working with enterprise clients across financial services and healthcare, the most frequent post-remediation incident type is a regression introduced under time pressure. Security-labelled issues are frequently escalated to senior management, creating pressure to deploy fixes faster than the test cycle would ordinarily permit.

This pressure is compounded by the sheer scale of the problem. The NIST National Vulnerability Database contains over 335,000 CVEs as of early 2026, with approximately 4,800 new entries arriving each month. According to research by the Kenna Security and Cyentia Institute (Prioritization to Prediction, Volume 3), the typical organisation only fixes about 10% of its vulnerabilities in any given month - meaning the backlog compounds with every release cycle.

Regression testing is the process of re-running functional and non-functional tests to ensure that previously developed and tested software still performs correctly after a change. When applied to vulnerability remediation, it closes the gap between "the security issue is fixed" and "the application still works."

Should You Treat a Vulnerability Fix Differently from Any Other Defect?

There is often an approach taken when dealing with vulnerabilities that because they are security related, they are somehow different from other issues and defects - that we need to "just fix it and get it in". This is understandable to some extent, because security-labelled issues are frequently escalated to senior management, creating pressure to deploy fixes faster than the test cycle would ordinarily permit.

The view needs to be changed to "fix it, test it properly, and then get it in." In other words, treat a vulnerability, at least from a testing perspective, in a similar way to other functional and non-functional defects.

The economic case for this discipline is well established. Research tracing back to Boehm and Basili's "Software Defect Reduction Top 10 List" (IEEE Computer, 2001) - and reinforced by a NIST-commissioned study on the cost of inadequate software testing infrastructure (NIST Planning Report 02-3) - shows that fixing a defect found in production costs significantly more than catching it during testing. The Consortium for IT Software Quality estimates the cost of poor software quality in the US alone at $2.4 trillion in 2022. Catching a regression before it reaches production is not a bureaucratic nicety; it is a straightforward cost-avoidance decision.

So, for each vulnerability there need to be two aspects to the testing:

  1. Have we properly remediated the vulnerability and tested it can either be downgraded or closed?
  2. Have we understood the fix that has been applied, assessed the impact of the changes, and regression tested appropriately? (This is the part which tends to get missed.)

What Testing Is Required After Receiving a Vulnerability Report?

So, you will have received a vulnerability report, probably from a security consultancy or an internal vulnerability scanning engine. This report will contain a number of different vulnerabilities each scored with a corresponding Common Vulnerability Scoring System (CVSS scoring system) score, published and maintained by FIRST.org (Forum of Incident Response and Security Teams).

Fixes to vulnerabilities come in many different forms, from simple configuration changes to complex code rewrites and underlying product version changes. An important aspect when thinking about testing of the fixes to the vulnerabilities - additional to the specific issue itself - is to understand the scale of changes required to enable the fix and to then impact and plan the execution of a test set which exercises that change appropriately.

The OWASP Testing Guide is a useful reference framework for structuring functional testing after security fixes, particularly for web applications. It provides a structured approach to verifying that security controls are in place without inadvertently breaking application behaviour.

To remediate the vulnerabilities you have been passed, someone needs to:

  • Prioritise the vulnerabilities into an agreed order (also known as triage).
  • Understand the fixes required and impact assess the changes.
  • Engage with the people and teams needed to do the actual fixing.
  • Engage with the people who will test that the fixes have been successful.
  • Engage with the people who will test that other parts of the applications have not been regressed as an unintended consequence of these fixes.

All of these actions align with what would typically happen in any of the other test phases in play, so we should try and think of the process in similar terms to other issues we fix and test as part of standard processes for maintaining and testing applications. By adhering to a test process we can start to get alignment with what we likely already have in place for testing efforts elsewhere in the company, in areas such as Integration Testing, User Acceptance Testing (UAT), Operational Acceptance Testing (OAT), and Performance Testing.

How Should You Prioritise Vulnerabilities Before Starting Remediation?

First thing we need to do is prioritise the vulnerabilities in the order which we want to resolve them. This ordering is often done by descending order by CVSS score. However, this ordering is somewhat flawed as "out-of-the-box" vulnerabilities are not given business contextualisation.

NIST SP 800-40 Revision 4 - Guide to Enterprise Patch Management Planning (April 2022) frames the patch management process as a risk-based discipline: organisations should define their own remediation Service Level Agreement (SLA) tiers aligned to severity classification, with a clear risk ranking process underpinning them. This is the foundation of a mature vulnerability remediation process.

Ideally, we recommend forming a Vulnerability Triage Group (VTG) - an internal governance body drawing on security, development, and business stakeholders - to apply business context to CVSS findings. This is a recommended internal governance construct, not a universally codified industry standard, but it reflects the multi-disciplinary approach NIST SP 800-40 Rev 4 advocates. Without a VTG, this prioritisation typically takes place best when the security consultancy producing the report is in discussion with the person responsible for the remediation, and with the network or application teams who can give more context to the vulnerability.

CVSS Scoring as a Starting Point

The table below shows a practical severity-tier model that aligns CVSS score ranges with remediation SLAs and regression scope. The SLA timeframes reflect common enterprise patch policies derived from NIST SP 800-40 Rev 4 risk-based principles and PCI-DSS v4.0.1 Requirement 6.3.3 (which mandates that critical patches are applied within one month of release for in-scope system components).

Applying Business Context with a Vulnerability Triage Group

For example, there may be a high vulnerability - one of five high vulnerabilities in the report - but one which applies to a server holding business-critical data. Now that we understand the criticality of the server upon which the vulnerability resides, this issue may become the natural candidate for priority remediation. A real-world illustration of why this matters: a CVE-rated vulnerability in a third-party library (similar in nature to CVE-2021-44228, the Log4Shell disclosure) can affect the same library across dozens of application components, meaning a patch to a single dependency may require regression coverage across multiple systems simultaneously.

This process of understanding the vulnerabilities within the organisation context is a good way to order our efforts and allows us to start getting the remediation plan together, with associated timelines for completion.

How Do You Coordinate Fixes and Testing Across Internal and Supplier Teams?

OK, let us assume we now have our agreed priority order which we are going to use to fix the vulnerabilities. The next step is to identify the people, the teams, the process, and the departments who are going to be impacted by these remediations.

Let us apply a working example to demonstrate some of the things to be considered.

Let us assume an organisation has a web application which has been developed by a third party. This third party develops the code and delivers into the receiving organisation as and when requested.

There are several things to think about when engaging them. First up we need to establish whether there is a Service Level Agreement (SLA) in place for them to resolve security vulnerabilities. There may also be a commercial discussion which is required, as they may have introduced the vulnerability themselves through poor code hygiene that violates OWASP Secure Coding Practices - inadvertently introducing security weaknesses during development rather than through deliberate negligence.

Suffice to say there are many elements to that supplier management which come into play, but for our purposes when considering regression, we need to establish what level of functional testing they will perform after they have resolved the vulnerability.

They may not have any agreement to do any testing, so it is well worth finding out at this stage such that you can plan to enhance the level of testing you perform after they have delivered the code back to you.

Unfortunately, in a lot of organisations, the people dealing with all things security related often have little or nothing to do with the teams who perform the other forms of testing against applications. Especially in larger organisations there are likely to be teams in place whose responsibility it is to test applications from both a functional perspective and from a non-functional, performance, and resilience perspective. We need to use the expertise and test automation capabilities of these teams to deliver our remediation activities quickly and safely into the live estate and avoid promoting code which may well fix the actual security issue, but break large swathes of the application in doing so.

A mature vulnerability management programme requires that these teams are engaged from the moment a vulnerability report is received - not after the fix has already been delivered.

This is what we are trying to avoid. Do not throw a change into production because "it's a security issue", without testing what else may have been impacted by the fix.

What Does an Effective Regression Test Process Look Like for Vulnerability Fixes?

The key to being able to fix and test quickly is to have an automated test set you can rely on to give you significant coverage and speedy execution times. While best practice is to maintain automated regression packs - and elite-performing organisations achieve this according to the DORA Accelerate State of DevOps research - many teams still rely on manual regression execution. In either case, the regression step must not be skipped.

Automated Regression Packs

We have taken some code; we have tested the vulnerability has been remediated. All good so far. We however have no idea what damage might have been done in the attempts at resolving that defect, as our supplier may not have done any level of regression testing.

If we are mature enough to have a DevSecOps (Development, Security, and Operations) practices approach where security is automated into our change process, and we have tools like Jenkins CI/CD (an open-source CI/CD - Continuous Integration/Continuous Delivery - orchestration tool) automating functional and non-functional automation, then great: we have a mature test process which includes security and we have likely mitigated the risk somewhat. (Also check what your suppliers are doing in this regard - do they do anything at all?)

DORA research shows that elite-performing engineering organisations achieve change failure rates below 5%. Lower-performing teams - those without mature test automation - can experience change failure rates well above 30%, meaning more than one in three changes causes an incident or requires urgent remediation. In a vulnerability remediation context, where the pressure to ship quickly is already elevated, starting from a high change failure rate baseline is a compounding risk.

What to Do When You Have No Automation

If, like many organisations, automated regression is not yet in place, we need to look to whatever automated functional test capability exists to give us assurance that the application functions as expected. We need to engage the test automation teams and ask them to run their regression packs - either over the whole application or, more likely, focused on the area of change. If no automation exists at all, a structured manual regression pass against the affected user journeys and integration points is not optional; it is the minimum acceptable due diligence before promoting any security patch to production.

What Exit Criteria Should You Apply Before Promoting a Vulnerability Fix to Live?

At the point when we have tested both the specific fix and run a level of regression over the applications, we need to look to an exit from the test phase.

Exits from test serve several purposes, one of which is particularly useful for the person responsible for vulnerability remediation: it allows us to document clearly what testing we did - and importantly, what we did not do. This is particularly important as it is nigh on impossible to test everything, and it allows a good response to the question which always seems to be asked: "Did you test everything?"

Exit criteria documentation also serves a compliance function. PCI-DSS v4.0.1 Requirement 6.3.3 requires that all in-scope system components are protected from known vulnerabilities by installing applicable security patches, with critical patches applied within one month of release. Formalised exit criteria reports are the evidence that this obligation has been met and are invaluable when a Change Advisory Board (CAB) or an auditor asks for proof of process.

Minimum Exit Criteria Checklist

Some simple exit criteria might be:

  • Were all our planned tests executed?
  • What was the test pass rate and is this acceptable?
  • Have any issues been raised in the relevant defect tool and assigned for resolving?
  • Were any issues introduced as part of this build and found in regression testing - can they be carried forward into a further release, or do they need to be resolved as part of this build?
  • Has the actual security issue been resolved?

Hopefully, this is mandated in your organisation anyway as a required input to a Change board. It is also worth formalising exit criteria as a standing clause in supplier SLAs - so that any third-party delivering a fix is contractually required to confirm what testing they performed before handover.

How Do You Build a Regression-Safe Vulnerability Remediation Process?

The pressure to promote a vulnerability fix quickly is real - but speed without regression testing trades one risk for another. Build the habit of treating every vulnerability fix as a change that must earn its way through the test process before reaching production.

The organisations that remediate fastest are not those that skip regression testing - they are those that have invested in the automation infrastructure that makes regression testing fast. A mature test automation estate, combined with formal exit criteria and change board documentation, turns vulnerability remediation from a high-risk fire-drill into a controlled, repeatable process.
